Forehead or Brow Lifts

In the forehead area, the aging process manifests itself in drooping eyebrows, hooding over the eyes, forehead furrows, and frown lines. Many people feel that these age-related characteristics make them look perpetually tired and unhappy. Patients often feel bothered by multiple forehead lines as well as lines on top of the nose and between the eyebrows. Many patients modify their hairstyles to hide these age-related changes.  Patients are often surprised at how much younger, brighter and refreshed they appear after their Brow Lift procedure.

If you are wondering what this procedure could do for your appearance, stand in front of a mirror and place the palms of your hands at the outer edges of your forehead above your eyebrows, then gently draw the skin upwards to raise the brow and forehead area. This will give you an idea of how effective a Brow Lift procedure would be in reducing your signs of aging.  The result of this procedure is a more alert, youthful appearance.

The Procedure
During the Brow Lift procedure, the muscles and skin that cause drooping and sagging are removed or altered to smooth the forehead, raise the upper eyelids and minimize frown lines. The placement of the incisions depends on the patient's surgical needs: 2 short incisions, 1 longer incision across the top of the head, or endoscopically. These alternatives and the implications of each are discussed with the patient in detail during the consultation process.

Brow Lift surgery reverses the effects of time, gravity, overactive muscle function, and genetics in the forehead region.  This procedure is commonly requested by both men and women ranging in age from their 20s to late 70s.  Many younger patients undergo this procedure to correct genetically low brows and hooding over the eyes.  If excess skin is present in the upper and lower eyelid area, eyelid surgery (Blepharoplasty) is often indicated in combination with the Brow Lift procedure to entirely refresh the upper face.

Dr. Barr uses 3 different surgical techniques to lift the brow: 

  • Endoscopic Brow Lift
  • Classic Hairline Brow Lift
  • Classic Bicoronal Brow Lift

    During any of the above Brow Lift procedures, the muscles and skin that cause drooping and sagging are removed or altered to smooth the forehead, raise the upper eyelids and diminish frown lines.  The technique chosen will be determined by individual patient characteristics and personal preferences. 

    Endoscopic Brow Lift
    This technique is the least invasive and involves five very short incisions, which are hidden in the hairline. The forehead skin is lifted and the underlying muscles are modified.  The endoscope, which is a long, thin device connected to a television monitor, facilitates visualization during the procedure and ensures proper realignment of the skin and muscle.  Sutures behind the hairline secure the new brow position.

    This surgical technique is commonly used in patients in their 20s to early 40s and is ideal for patients who have significant midline creases, moderate brow droop, but minimal transverse lines.

    Classic Hairline Brow Lift
    This technique involves an incision that is hidden as it follows the hairline in a staggered step pattern.  This technique is used in patients who have excessive brow skin and significant forehead lines, in addition to midline creases and brow drooping.  This technique also allows shortening of a forehead that the patient feels is too tall. 

    Classic Bicoronal Brow Lift
    This technique involves an incision which is completely hidden within the hair.  This incision is behind the hairline and is unnoticeable when healed. 

    The best candidates for this technique are patients who have significant forehead lines and brow drooping, along with multiple midline creases and a relatively short forehead.  By using this technique, Dr. Barr is able to provide a more attractive forehead height while still completely hiding the surgical scar.

    The Post-Operative Course
    Brow Lift surgery is usually performed under general anaesthetic, although in some patients, IV sedation combined with local freezing can be used.  Patients do not require an overnight stay in the Surgicentre unless the procedure is combined with a Rhytidectomy (Face Lift), in which case patients are kept overnight.  

    Patients are discharged with a dressing in place, which is left intact for 3 to 4 days.  Patients are instructed to keep their head elevated for 2 to 3 days. Bruising and swelling usually begin to disappear in about 1 week. Patients will be up and about in 1 or 2 days, but should take it easy for a full week. Most patients are back to work within 7 to 10 days. Patients are seen in follow-up at 3 to 4 days post-operatively for dressing change and wound assessment.  Sutures are removed at 7 to 10 days depending on the surgical technique used.  Patients are to limit their activity for the first 4 to 7 days post-operatively, but can participate in light activities after 1 week.  Full activity, including sports, can be enjoyed after 3 weeks have passed.

    Concurrent Procedures
    All facial rejuvenation surgery, including Upper and/or Lower Lid Blepharoplasty (eyelid surgery), CO2 Laser, TCA Peels and Rhytidectomy are commonly combined with the Brow Lift procedure.
